Open your Arizona state return in your tax software.
Continue until you reach Tax Credits (often called Other Nonrefundable Credits).
Select “Qualified Charitable Contributions (Form 321).”
Enter the charity name (Jam Pak Band), QCO code (20933), and donation amount.
Finish and file your return.
